TPS Waiver Form/ Application to Play

Everyone who plays must fill out the Tallahassee Paintball Sports Application to Play and the waiver form. Those under 18 must have an adult signature as a Guarantor.

Please print, fill out, and sign before your scheduled time to help speed up the check-in process. Blank forms are available for pick up at TPS field on the day of play.
